After you have signed up for your STAR date, review the following (If you haven’t yet, click here to sign up) :
Some forms may require you log in to your Viterbo student account before completing them. If you are prompted to log in after clicking on a link, enter your Viterbo username and password. If you are unsure of your username and password, please visit viterbo.edu/username.
Required before students register for classes during STAR or absentia registration:
- Student Financial Responsibility Agreement - Regardless of age or financial aid offer, all new students must read and sign the Student Financial Responsibility Agreement (SFRA) in VitNet Self Service.
- In VitNet Self Service, find the "Academics" section on the left side of the page and scroll to "Student Financial Responsibility Agreement (SFRA)" to sign the agreement. This form also asks for your consent to receive student account information electronically and asks you to acknowledge your assumption of risk for off-campus programs/activities. .
- Parent/Guardian Financial Responsibility Agreement - If a student is not yet 18 years old, a parent/guardian must also electronically sign the Parent/Guardian Student Financial Responsibility Agreement. After the student completes their SFRA, their parent/guardian will receive an email at the parent/guardian's email already on file to electronically sign the online agreement.
- Request Final High School Transcripts - After your final grades have been posted, an official final transcript that includes a graduation/diploma date must be sent to Viterbo Admissions. Final transcripts should be sent directly from your high school either by U.S. Mail (below), using an electronic transcript delivery service (like Parchment), or by email directly from the high school.
- Submit your ACT/SAT results to transcripts@viterbo.edu or complete the Viterbo Math and English Placement exams in Moodle. English and Math Placements will be required unless a student has documented with Viterbo Admissions one of the following: screenshots of ACT or SAT scores that include student name, date of the exam, and relevant scores, college transcripts showing previously completed college credits with a C or better in English or Math, or acceptable exam scores from select Advanced Placement or International Baccalaureate exams.
- Viterbo placement exams will be available online via Moodle within one week of new students signing up for STAR.
- Viterbo Math Placement exam note: Students may use a calculator on this online exam. Alternatively, an online scientific calculator is provided.
Additional, optional items:
- Request Final College Transcripts - If you have taken any college coursework, after your final grades have been posted, official final college transcript(s) must be sent to Viterbo Admissions. Final college transcripts should be sent directly from the institution initially awarding the credit. Viterbo cannot accept college courses listed on high school transcripts.
- College Board/AP or International Baccalaureate Credit - Any student who has earned credit via one of these programs is reminded that they must provide official test scores or transcripts to receive credit at Viterbo. Students should inform their Viterbo advisor of those courses at the time of registration.
- Accessibility Services - If you are requesting any type of academic or non-academic accommodations, submit the online form.
- Emergency Text Messaging and Hometown Newspaper - New students are encouraged to sign up for the emergency text message system and list your hometown newspaper (for dean's list, honors, and graduation announcements, etc.).
- Spanish Placement – We encourage all students who have taken Spanish in high school or that are heritage speakers to take the Spanish placement exam. Such students may earn 3 to 12 retroactive credits pending the outcome of the placement exam and earning a grade of C or better in their first university Spanish course above the 101 level. Students who have successfully completed a college Spanish course (with a grade of C or better) or have an acceptable score on an AP or IB exam prior to coming to Viterbo do not need to take the Spanish placement exam; however, official college transcript(s) or official exam scores MUST be submitted to Viterbo University.
Required prior to arrival in the fall:
- Health and Immunization Record - Required student immunizations include two doses of MMR and a booster dose of Tetanus-Diphtheria (Td) within the past 10 years. Polio and Hepatitis B are required for students in clinical health-related studies, but are also highly recommended for all students. Meningococcal (Meningitis) A & B vaccines are highly recommended.
- Should there be any objection to required immunizations, an immunization waiver form must be completed and submitted to Health Services (healthservices@viterbo.edu). Please note that declining immunizations may present challenges to required clinical placement experiences for some majors. If you have questions, please consult departmental handbooks and other available departmental policies for more information.

- Health insurance information submitted via this form – This includes the insurance carrier, policy number, and phone number. A photo of the insurance card can be submitted under ‘attachments’. If there is any missing information, the form will be sent back for completion/correction.
