• UG Academic Changes 
  • This form is used by undergraduate students to change, add or drop a major, minor, advisor, emphasis, catalog year, or graduation date change.
• Application for Academic Forgiveness
  • Students who reenter Viterbo University after a minimum of two years absence from Viterbo may apply for academic forgiveness of previous course work at Viterbo. 
• Academic Policy Exception
  • Students seeking an exception to any university academic policy listed in the University Catalog should complete this form to petition for that exception. 
• Transfer Course Request
  • Students seeking to complete coursework at another institution and transfer credit to Viterbo should submit this form for course approval.
• Outstanding Academic Advisor Award Nomination
  • This award is presented annually during the Rose Awards.
