Chronic Disease Community Needs Assessment
Partnership Grant staff and Viterbo Senior student nurses conducted a chronic disease needs assessment from September through November of 2009. The purpose of the needs assessment was to obtain information about chronic disease and access to healthcare from the target population of uninsured and underinsured citizens. The data was used to determine the priority health promotion needs of this population. New health screening, immunization, and education services will be implemented to meet these needs. A full report of the data collected can be accessed on the webpage using the report links.
Questionnaires
The information (data) was collected by administering 74 questionnaires to participants during 11 sessions held at 9 locations in Vernon County. Questionnaires were administered at locations where citizens receive other services such as: a) Bethel Butikk Food Pantry, b) Living Faith Church Food Pantry, c) New Hope Food Pantry, d) Viroqua WalMart, and e) Fortney Hotel.
