Outstanding Academic Advisor Award Nomination Form

The Outstanding Academic Advisor Award is presented at the Rose Awards Ceremony to an individual who has demonstrated qualities associated with outstanding academic advising of students as reflected in the mission statement, statement of core values, and advisor responsibilities.

Any Viterbo employee who serves in an official capacity as an academic advisor is eligible for this award. A minimum of two nomination forms must be submitted for an advisor to qualify for the award.

Please describe how your advisor has demonstrated any or all of the following criteria. Specific examples will support your nomination better than general comments.
